Services

Intake Sessions-  All intakes for psychological services are scheduled for 60 minutes.   In this session we gather information about development, health, family, mental health, educational, behavioral, and other relevant information about why you are seeking testing or counseling.  

Psychological Testing/Evaluation- is a comprehensive evaluation of an individual’s overall functioning with the goal of determining the most appropriate diagnosis/es and specific recommendations for interventions.  It is generally a series of three appointments:  the intake, the testing session, and the feedback session.   After getting authorization from insurance, testing is scheduled and will take from 1-3 hours to complete.  Testing may include intellectual, academic, attentional, social, and emotional functioning.  After completing testing sessions, the feedback session will be scheduled approximately three weeks later.  At the feedback session, I will discuss the results of testing, diagnosis/es, and associated recommendations. 

Psychological Therapy is both a way of understanding human behavior and helping people with their emotional challenges and difficulties.  Therapy typically starts with an evaluation of problematic symptoms and maladaptive behaviors that cause difficulties in a person’s relationships, school, work, social life, and health.  Specific psychotherapeutic strategies are employed to alleviate specific problems causing distress such as depression or anxiety.  The key to making progress in therapy is to learn about how your own beliefs and actions can help or impair your functioning and work to utilize the most effective strategies for your needs.  Therapy may be a brief process (8-16 sessions), or may last longer if there are multiple issues or challenges.  The process ends when the therapist and client feel they have met the desired goals and have no need to progress further.

Consultation Services

A variety of consultation services are available including:

  • Group presentations on mental health topics such as stress management, mindfulness, anxiety, or depression

  • In-person behavioral observations and programming

  • Consultation with schools and school districts to address challenges for students with special needs

Fees

Payment Options:  Please be aware that you are responsible for any copays, coinsurance, or other payments that are due at the time of your appointment.  Accepted forms of payment are cash or credit cards (all major cards accepted).

Insurances accepted: Optum, Tricare, BHS, Cigna, BC/BS, Aetna

Private Pay $175 per hour for Dr. Surdock- $75 per hour for graduate student clinicians

Missed Appointment Fee

Failure to show for a scheduled appointment, provide a valid reason for missing the appointment, or notify the office of cancellation/rescheduling at least 24 hours prior to the appointment time, will result in a $50 missed appointment fee. This fee will be waived for the first missed appointment or if there is a valid, documented reason (e.g., illness with a doctor's note). In case of illness, some sessions may be rescheduled to a teletherapy session if this is appropriate and covered by your insurance.
